Theodorus Willebrordus Maria (Theo) Mackaay is een Nederlands beeldhouwer, schilder en musicus die op 19 november 1950 in Utrecht werd geboren. Tussen 1972 en 1976 studeerde hij aan de Gerrit Rietveld Academie in Amsterdam en van 1977 tot 1979 ging hij naar de Rijksacademie van Beeldende Kunsten in Amsterdam.
Vanaf 1980 begon hij zijn carrière als professioneel schilder en beeldhouwer. Zijn figuratieve werk is geïnspireerd door Pablo Picasso, Marino Marini en Ossip Zadkine. In 1980 ontwierp hij de Gouden Kalf, een beeld dat bekroond is op het Nederlands Filmfestival.
Mackaay heeft diverse tentoonstellingen gehad in binnen- en buitenland, waaronder Galerie Morren (Amsterdam, 2015), Carlton Hotel (Cannes, 2015), Amstel Hotel in Amsterdam (2016). Mackaay is ook een muzikant die gitarist en zanger was in verschillende bands.
